COVID-19 is a global pandemic causing serious disease and mortality worldwide. People infected with COVID-19 can have a range of symptoms from asymptomatic to very severe. Due to limited availability of testing, mild and asymptomatic cases often go unreported. This prevents the public health community from accurately measuring the spread of the COVID-19 virus.

EpiPointe developed this survey to gather information on mild and/or unreported cases of COVID-19 within our global communities that have not required hospitalization. If you tested positive for COVID-19 or believe you were infected due to direct contact, and have not been hospitalized then we want to hear from you. Your participation in this survey will help scientists better understand how this disease spreads and develop methods to help prevent a pandemic like this in the future.
